The person concerned has the right to apply if he/she is an adult or one of the two parents, in case of a minor. For an event which has taken place before February 2018 of another municipality, a Birth Certificate with the new name and a copy of a court order is required. For males registered in the Mens’ Register, a certificate of registration in the Mens’ Register with the new name is required.

All electronic documents attached as supporting documents (birth certificates, certificates, etc.) must be digitally signed by the competent issuing authority or issued by gov.gr.
